Anti-Dandruff Shampoos: Short-Term Treatment or Lifetime Habit?

28 Jul 2026  •  Neemyukt Research Team

Every dandruff sufferer knows the cycle: the medicated shampoo works, the flakes vanish, you triumphantly go back to your regular shampoo — and three weeks later the flakes return, as if mocking you. This isn't the shampoo failing. It's a misunderstanding of what dandruff is.

Why dandruff comes back

Common dandruff is driven by Malassezia, a yeast that lives on virtually every human scalp. It feeds on scalp oil, and in susceptible people its by-products irritate the skin into shedding cells faster than they can be neatly cleared — hence flakes. Medicated shampoos knock the yeast population down; they don't remove the yeast from the planet, your genetics from your body, or oil from your scalp. Stop entirely, and conditions gradually return to what they were. In that sense dandruff behaves less like an infection you cure and more like a tendency you manage — the same way oily skin is managed, not cured.

First, though, make sure it's actually dandruff: dry-scalp flaking is a different problem with an opposite solution, and we've written a full guide to telling them apart.

The rotation strategy

The mistake is treating it as all-or-nothing — either daily medicated washing (which over-dries many scalps and leaves hair rough) or none (relapse). The sustainable pattern for most people is rotation:

PhaseWhat to do
Active flakingMedicated shampoo (e.g. ketoconazole 1-2%) two to three times a week, as per pack/dermatologist directions, until controlled — typically 2-4 weeks
MaintenanceMedicated wash once a week or fortnight; a gentle balancing shampoo on other wash days
Quiet periodsGentle shampoo only; resume the medicated wash at the first sign of return

The "other days" shampoo matters more than people think. A harsh sulphate wash strips the scalp, the scalp compensates with more oil, and more oil means more yeast food — quietly undoing the medicated shampoo's work. A mild formula with traditional scalp-purifying botanicals earns its place here: something like our neem-argan shampoo cleanses gently while neem and tea tree keep the scalp environment less hospitable between treatments. Leave any shampoo on the scalp for a minute or two before rinsing — contact time is most of the job.

Habits that tilt the field

Wash often enough that oil never accumulates for days — in Indian humidity that's usually every two to three days, a topic we've covered in how often to wash. If you oil your hair, apply it hours before a wash, not after one; parking oil on a dandruff-prone scalp overnight is feeding the yeast, as explained in champi, done right. Rinse thoroughly after workouts, and wash pillowcases weekly.

When it isn't ordinary dandruff

Thick yellow greasy scale, red inflamed patches, flaking that extends past the hairline to eyebrows or nose folds, intense itching, or any flaking with hair loss in patches — these point to seborrhoeic dermatitis, psoriasis or fungal infection, all of which need a dermatologist rather than shampoo roulette. Scalp psoriasis in particular is routinely mistaken for stubborn dandruff for years; persistent thick plaques deserve a diagnosis.

Frequently asked questions

Is it harmful to use ketoconazole shampoo every day?

It's unnecessary and drying. These are treatment products with directions for a reason — two to three times weekly is the standard pattern, tapering once controlled.

Can natural shampoos alone cure dandruff?

For mild, occasional flaking, a well-formulated gentle shampoo with scalp-purifying botanicals can be enough. For established dandruff, think of botanicals as the maintenance layer and medicated washes as the treatment layer.

Does dandruff cause hair fall?

Indirectly it can — chronic inflammation and vigorous scratching stress follicles. Control the dandruff and that component of shedding settles; if loss continues, read shedding versus balding.
